ESA Assessments

Your provider will assess your symptoms, needs, and whether emotional support animal documentation is clinically appropriate. The assessment fee covers the visit; it does not guarantee a letter or approval of a request.

Please provide any required forms, documentation instructions, or submission link before your appointment so your provider can review the requirements.

If appropriate, an ESA letter will be completed at the conclusion of your appointment. Requests requiring additional information or specific forms may need further review.

Verify benefits before booking. Insurance coverage for the clinical assessment does not guarantee coverage for letters or administrative paperwork. Applicable patient balances remain your responsibility.
