By accessing or using this website, you agree to these Terms of Use and our Website Privacy Policy. If you do not agree, please do not use this website.

1. About Amenity Healthcare
Amenity Healthcare, PLLC (Amenity Healthcare) provides telehealth-based healthcare services to eligible patients who are physically located in states where its clinicians are licensed or otherwise authorized to practice. At this time, services are offered to eligible patients located in Utah and Iowa, subject to provider licensure, clinical appropriateness, insurance participation, applicable federal and state law, and practice requirements.
Amenity Healthcare is a telehealth-focused practice. Mailing addresses listed on the website may be used for administrative or business purposes and may not represent walk-in clinical locations.
2. Website Content Is Not Medical Advice
Website content is for general educational and informational purposes only and is not a substitute for individualized medical advice, diagnosis, treatment, or emergency care. Use of this website alone does not create a provider-patient relationship.
3. Emergencies and Crisis Situations
Amenity Healthcare does not provide emergency or crisis services. Do not use this website, standard email, text messaging, voicemail, portal messaging, billing email, or appointment request tools for emergencies or urgent clinical concerns. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. If you are experiencing suicidal thoughts, emotional distress, or a mental health crisis, call or text 988.
4. Telehealth Services
Telehealth may involve secure audio, video, electronic, or other remote communication technologies. Telehealth has benefits and limitations and may not be appropriate for every condition or situation. Your provider may recommend in-person care, urgent care, emergency care, laboratory testing, vital signs, medical-record review, referral, or a higher level of care when clinically appropriate.
You must be physically located in Utah or Iowa, as applicable to the treating clinician’s authorization, at the time of the visit. You may be asked to confirm your location, phone number, emergency contact, and local emergency resources.
5. Patient and Website User Responsibilities
You agree to provide accurate, complete, and current information when requested and to maintain the confidentiality of login credentials used for scheduling, payment systems, patient portals, or other online tools. You may not misuse this website, attempt unauthorized access, interfere with website operations, upload malicious code, impersonate another person, scrape the site without authorization, or use the site for unlawful purposes.
6. Communications
Standard email and general website contact forms may not be encrypted and should not be used for urgent or sensitive clinical concerns. Patient-specific clinical questions, medication concerns, and sensitive health information should be communicated through the secure patient portal or another secure method designated by Amenity Healthcare.
7. Prescribing Policy
Amenity Healthcare does not guarantee that any medication will be prescribed. Medication decisions are based on clinical judgment, patient safety, diagnosis, treatment history, available medical records, applicable prescription-monitoring requirements, required monitoring, telehealth appropriateness, and applicable federal and state law. Controlled substances are prescribed only when clinically appropriate and legally permitted.
8. Payments, Insurance, and Patient Responsibility
Patients are responsible for applicable charges, including self-pay fees, copays, coinsurance, deductibles, denied or non-covered services, and balances permitted by law and payer contracts. Verification of insurance benefits is not a guarantee of payment.
9. Self-Pay Fees
Current self-pay fees are disclosed through scheduling, intake, the patient portal, Good Faith Estimate, or other practice materials. Fees may change prospectively. Outside services such as medications, laboratory testing, pharmacy charges, emergency or hospital care, psychological testing, specialist care, and third-party services are not included unless expressly stated.
10. Late Cancellation and No-Show Policy
Appointments must be cancelled or rescheduled at least 24 hours before the scheduled appointment time. A $100 late cancellation/no-show fee may be charged for missed appointments or cancellations made within the 24-hour window, except where prohibited by law or payer contract.
11. Good Faith Estimate Notice
Uninsured or self-pay patients may have rights under federal law to receive a Good Faith Estimate of expected charges for scheduled or requested non-emergency care. For questions, contact info@amenityhealthcareonline.com or (813) 773-1932.
